What does a bulging soft spot on a baby's head mean?
A soft spot on the head that looks swollen or bulging is a reason to call your pediatrician right away. Both the AAP and the NHS list a swollen or bulging soft spot, the fontanelle, among the signs that need a call.
Call your pediatrician right away (UK: your GP or NHS 111) if your baby:

Soft spot on top of head looks swollen
At 3 AM
The soft spot is the gap between the skull bones on top of the head. You have seen it every day, which is the point: the sign is a change from what you know.
Look with the baby calm and held upright, in good light, and compare with how it looked this morning. There is no need to press on it; look, compare, and call if it is swollen or bulging.
Have these ready: the baby's age in weeks, what the soft spot looks like compared with usual, any fever, any vomiting, how the baby is feeding, and whether they are as alert and easy to wake as usual.
If the baby is also hard to wake, breathing differently or has changed colour, those are emergency signs with pages of their own. Call 911 rather than the pediatrician.
Note the time you noticed it in the log, with a photo if you can take one in good light.
Where the sources differ
The AAP's newborn appearance page says swollen. The NHS says swollen or bulging. They agree on the sign. The NHS lists it on its urgent-help page; this page keeps the pediatrician-first action the app uses.
